I need to take a cough syrup that contains a small percentage of alcohol as a preservative or solvent. Is alcohol in medicine halal according to Islamic law? Is it permissible to consume such liquid medications?

When addressing the question of whether is alcohol in medicine halal, we must first recognize that the general principle in Islam is that the consumption of intoxicants (Khamr) is strictly prohibited. However, contemporary Islamic jurisprudence councils (such as the International Islamic Fiqh Academy) have thoroughly addressed the issue of synthetic alcohol used in modern medicine.

If the alcohol used is not derived from grapes or dates, and the amount is so minuscule that it does not intoxicate even if consumed in large quantities, and there is no suitable Halal alternative available, it is permissible to use such medication for treatment. The alcohol in these medicines acts merely as a solvent and loses its original intoxicating property in the mixture—a process known in Islamic jurisprudence as Istihlak (dilution/absorption) or Istihalah (chemical transformation).

The Principle of Medical Treatment:

Islam highly encourages seeking medical treatment while avoiding clear prohibitions. It is always recommended for a Muslim to seek pure, alcohol-free alternatives first. However, if a qualified doctor prescribes a specific medicine and no pure alternative exists, the allowance is granted based on the principle of necessity.

قَالَ رَسُولُ اللَّهِ صَلَّى اللَّهُ عَلَيْهِ وَسَلَّمَ: "إِنَّ اللَّهَ أَنْزَلَ الدَّاءَ وَالدَّوَاءَ وَجَعَلَ لِكُلِّ دَاءٍ دَوَاءً فَتَدَاوَوْا وَلَا تَدَاوَوْا بِحَرَامٍ"
Translation: The Messenger of Allah (ﷺ) said: "Allah has sent down both the disease and the cure, and He has appointed a cure for every disease, so treat yourselves medically, but use nothing unlawful."
